 27-year-old plus(?) Moen 2-Handle Widespread Sink Faucet

I need to replace the cartridges for my old Moen sink faucets. I've looked into the Moen's site but can't find the exact model. Mine seems to be similar to the model 4962. Please help me to identify the model number. What are the steps to remove the cartridges? Do I have to remove the escutcheons after removing the knobs? Can I install the new trim from other brands? Thanks.

NO!!! You get the "stem removal tool", (the part number is 14272), then unscrew the white piece, use the tool to loosen the stem, pull the stem out, then put the new one in in the reverse order. The stem model number is #1224.
