Triangle tank, no name, has number stamp 2209 something something. Lid stamped 2892 then some weird symbol I cannot make out.

the overflow is part of the tank. all other parts are readily available. |

it is an Eljer "Triangle". You can purchase a new "flush assembly" for it, and the fill valve is common and any fill valve will work in it. |
