I am thinking of adding this "thermostatic mixing valve" to our electric storage-tank water heater ...

... in order to extend the quantity of available hot water. The idea is to turn the water heater up to 140 degrees (or higher) and set the mixing valve to 120 degrees. This is how the product is advertised.

How concerned should I be about safety, about the valve accidentally passing hot water directly from the water heater, at 140 degrees (or whatever I set it at), instead of 120 degrees, and causing injury to occupants ?

over a long period of time, that is a concern. but right out of the box they are dependable.

..... and they 'tend' to fail slowly giving ample 'warning' ..... IMO

What kind of warning ? Water too hot ? I've heard it can be mineral buildup causing the problem.

yes, the water will 'generally' become gradually too hot

The instructions for my Honeywell-Residio one say, once a year or so, to tweak the temperature setting slightly, and if you don't see the temperature of the mixed water change immediately, the thing needs servicing.
