I am doing a basement bathroom (shower, toilet and sink -> sewage ejector pit).
Here is a picture of a non-glued mockup of what I was planning to install under the concrete.
Any and all comments welcome.

Many thanks the folks on this site who have already addressed my previous mistakes and answered my questions.

I live in New York and I believe that means IPC for the inspection.

Your Y for the toilet vent is backwards

If those are the actual branch distances my code says the toilet does not have to be vented, you will need a vent where your end of the line cleanout is, and no pipe smaller than 2" should be underground

Sorry, those are the pipe ID sizes. The total bathroom is 8.5' long and 5' wide.
So, I can remove the toilet vent and the toilet would be wet vented??

I don't understand. Are you saying a vent by the cleanout OR a vent by the sewage ejector pit?
If you are referring to the sewage ejector pit end.....then, I agree and am planning to vent that in a new 2" vent pipe vertical into the attic where it will join an existing vent just before exiting the roof.

Everything here is 2" or greater except for the sink 1.5" p-trap (not shown).

If the toilet branch is within 8' off the main you do not have to vent it, a vent 3" vent at the end of the line to open air will work just make sure the penetration is 10' away from any windows

this is how i would do it.
forget the cleanout. you will never use it.
run 3 inch to the toilet and all else will be 2 inch.
use the toilet vent to drain the shower and the lav.
classic wet vent set up..

the 2 inch Y must be within 6 feet of the shower trap

Make sure the drainage system will below the bathroom fixtures so that they can drain well. Here are some more info of building bathroom in the basement.
