 Help Identifying Toilet to Replace Tank Lid

In an unfortunate accident the other day I broke the lid to the tank of my toilet. At first I thought it would be easy to replace but I'm having trouble identifying the make and model of toilet. The numbers on the inside of the tank are almost worn down so I know it's an older model. I have posted a few pictures of the numbers, broken tank lid, and pictures of the whole toilet from very angles on the site below. I've reached out to a couple of salvage sites and one was unable to identify the make/model and another said it was an Eljer 151-0100 but I would love additional opinions before ordering a replacement.

I can post additional photos or provide measurements if there is additional information that would be helpful. Just let me know.

That bowl shape is similar to an American Standard Cadet. but not sure if its number was F-2122.

i don't believe the cadet had that kind of flush mechanism.
the square-ish tank make me think gerber?
