 shower arm connection to drop ear elbow

Our plumber left what I think is a nipple attached to the drop ear brass elbow for the rough-in.
I want to install a long shower arm to hold a fixed rain shower-head.
The backing for the tile is already in place now (wasn't able to take a recent photo).
My question is, do I unscrew the nipple and screw in the shower arm in the small opening?
I've tried a pipe wrench to get the nipple off but it wouldn't budge.Should I try an internal pipe wrench?
I wanted to make sure I was doing the right thing before going further.
I also have the same situation for the handheld shower.

yes, the nipple unscrews and the shower arm just screws into the drop ear 90..
the plumber just made the test nipple too tight.
it should unscrew with a bigger wrench.
same goes for the handheld test nipple.
P/S, put 3-4 wraps of teflon tape onto the shower arm threads then dab some good pipe dope ontop of that. kind of a belt and suspenders approach but you can't be too careful with that hidden connection..
