 Moen 1222 Cartridge install issue

Hi all-

I just replaced a bad cartridge in a shower handle. It's a Moen 1222 cartridge. When working properly, off position should be at 6 o'cock, and turn counter-clockwise to turn on, all the way around to 9 o'clock to be at fully hot. You probably all know this much....

problem is when I installed the cartridge (with the H/C at the top of the cartridge) and put everything back together, the water was running full blast at the 6 o'clock position. Turn counterclockwise, and at 12 o'clock the water turns off, then if you keep going to the 9 o'clock position it starts again.

so I took it apart, flipped the cartridge upside down (H/C at the bottom this time) and same thing. It's off in the 12 o'clock position. Turn it clockwise to the 6 o'clock spot and get cold water, turn it counter-clockwise to the 9 o'clock and get hot water.

I'm missing something......

any advice?

The notch in the tip of the shaft should be facing down(off position) before installing the handle adapter.

Is the sleeve and limit stop installed?
