Fresca, is the importer of your FTL2377, 1.28 gpf toilet made in a country, some where on this planet, but we can't say. The finished product was shipped into Long Island/Queens, New York to a company that wishes not to be named at this time. They don't have a direct telephone number but will only communicate with the outside via a email or fax number. 347-402-1053. What might be more interesting is that there are no reviews or MAPS info on your toilet so it is most difficult to offer you advice.
The import, paperwork says the toilets are shipped with seats, instructions and wax ring. Did you purchase your toilet at a flea market with open box or directly from a company that sellers world made imports?
Best Wishes

The ones I have heard about with two holes had a "non overflow" passage that drained to the second opening.
