it might be as simple as keeping the trap close to the center and piping it like a double bowl kitchen sink.

if there are no drawers in the middle, just pipe it like a double bowl kitchen sink. BTW, your trap is installed backwards.

There is drawers in the middle of cabinate, but there is some space at back of drawers.

then stack 2 sanitary tees inside the wall. the top one faces out and the one below faces to the left with a short piece of pipe and a 90..

Can I use 1 1/4 off the 2 inch line, or is 1 1/2 recommended?

Code does allow 1 1/4" pipe to be used in that situation, but you must also use drainage fittings, not short sweep pressure fittings.

Is an alternate to use a 4-way be instead of 2 tees? Also, should the line going to the new drain be exactly level or can it slope down a tiny bit?

It might be best to use a back to back fixture fitting, or a double sanitary tee rather than stack 2 tee's. The piping needs to be sloped up as it leaves the tee.
