Photo below for any others with this sort of problem. Lessons here were:

Even if handles appear to be one piece - make sure whether there is a removable cap with fastener below. Manufacturer (probably California Faucets) cleverly built chrome handles with machined o-ring caps that fit so well, the joint was not discernible without magnification. Tip from this site gave me the urging I needed to find joint and remove the caps. Also, flared chrome trim below knob also threaded off.

I also appreciate the warnings about not using a giant pipe wrench to force faucet valves apart with handles attached. Too much reading on my part about handles hopelessly stuck on faucets by others.
