 delta handheld shower falls off

This might be a stupid question. We're on our second Delta handheld showerhead. Both the first one (which lasted several years) and the second one (which I hooked-up less than a month ago) have the nasty habit of falling out of the wall mount, which is a potentially dangerous situation.

For the first one, I eventually used a hose clamp to attach a small chain that I then hooked into the wall. If the showerhead fell, the chain would catch it before it crunched on someone's head. Or foot.

I was hoping the second one wouldn't need the same homemade solution.

What is up with Delta handheld showerheads that they're designed to fall out? These are the models:

1. Traditional Style Five Spray Massage Hand Shower Unit [www.deltafaucet.com]

2. 7 Function Hand Shower [www.deltafaucet.com]

Is there a solution other than to rig up the hose clamp and chain again?

Is the correct end of the hose at the handshower ?
Is the swivel tight enough that the bracket stays upright ?

The bracket AND the handheld are tapered so they would have to be lifted to come out of the bracket. There is something else happening which we cannot tell without seeing it.

Thanks, guys. What seems to happen is it works its way up, like it doesn't seat all the way into the mount. Plus, it's all chromed which makes it slippery when wet. As if the handheld part isn't heavy enough. Bump the hose, water pressure on and off, etc. and it starts to edge out. Also, if someone adjusts the shower head downward, it might get too horizontal and slip. Maybe if I hung one of those donut weights like is on the hose under the kitchen sink.

Maybe it's operator error. Just seems to me that an adustable, handheld showerhead should be safely adjustable.

what you are describing is NOT what normally happens. Is the bracket the proper one for your handheld. They are NOT generic.

It's the bracket that came in the package. ??

Maybe I have the hose ends reversed. I'll check, but they look identical to me. They hose ends also leak. I have them as tight as possible (instructions recommended not to go too tight) and I used teflon tape. Still leaking.
