I have two Kohler Elongated Cimmaron toilets, purchased from a big box store about 2 years ago. Both toilets have the same problem. On many occasions the flush valve arrangement rotates and jams the flush valve so the water never stops filling the tank. The flush valve is a 3" diameter cylinder about 8" long that the flush lever attaches to with a chain. The chain lengths were set at the factory and don't appear to be too long or short. The flush lever is sitting on the left side wall of the tank. When flushed the flush lever rod pulls up on the chain which pulls the 3" cylinder up and starts the flush. When the cylinder falls it seals. Any ideas as to what causes the cylinder to randomly rotate and jam the chain and therefore not seat?

The chain should be short enough so that the cylinder doesn't rotate far enough to tangle the chain. Are the handles in good condition ? Sometimes the lower stop will break, allowing the handle to fall down further than normal allowing too much slack in the chain.

The lever stops are perfect. I will try shortening the chain by one link and if that doesn't work I will call Kohler. I bet this is a common problem with the newer Kohler low flush toilets.
