A couple of drains are slow in my house, so I opened a clean-out on that line. I could hear air/gas hissing as I slowly unscrewed the cap. There wasn't wastewater at the cleanout so I don't think it's totally blocked. My guess is that the vent is plugged up too, and that's why I heard the hissing. What do you think?

a drain system should never be under pressure unless it is a force main. if you heard a hissing sound then you should definitely look into the vent system and obviously the blockage in the main further down the line.

It may just have been the air trapped in the cleanout riser.

Absolutely POSITIVE a vent is blocked! Drainage systems are at atmospheric pressure and breath better than you do...if the vent ain't blocked.
