As toilets went from 3.5 gpf to 1.6 and then 1.28 did the plumbing codes change to allow the waste system to better accomodate low flow toilets? If I built a new house now with all 1.28 gpf toilets, would the waste system be designed differently than if I used 1.6 gpf toilets?

MA code has not changed. we still are allowed 3 toilets on a 3" drain. after that we must increase to 4". it makes no sense to me.

WI took away their 3 toilets = 4"...its now all on DFU...so you could have 9 toilets on a 3" sewer
i dont think it will matter with pvc, but i hear from drain cleaners that some older houses now need their main drains snaked a couple of times a year because there isnt enough water being flushed to push the poop down...the poop is sitting while the water goes to the city sewer
